Our Approach to Home Healthcare

We provide 24/7 care, including on weekends and national holidays, for all patients that conclude scheduled house call contracts with us.

This service requires multidisciplinary cooperation, such as visits from nurses and pharmacists.

1. In the case of home healthcare services, initial contact is via a visiting nurse just like pressing a call button if hospitalized. Then, if necessary, the visiting nurse will contact a doctor, who will make a diagnosis over the phone or through an unscheduled house call. Nursing visits are also available 24 hours a day, every day.

2. We provide periodic scheduled house calls based on the patient’s condition; such as once or twice a month for patients in stable condition, and once or twice a week for those whose condition is unstable, and daily for those in severe condition. Regular visits are called scheduled house calls, while emergency visits are called unscheduled house calls.

3. A pharmacist normally accompanies the nurse on visits, to check details regarding any medication (medication status, dosage omissions, issues concerning medication, types of medication, dosage and timing, etc.) Pharmacist visits are also available 24 hours a day, every day.

Home Healthcare Details

Differences between Hospitalization and Home healthcare

If we become seriously ill, we are usually hospitalized. Then, once we recover, basically, we are discharged from hospital, and try to get back to normal. However, some people suffer from uncurable diseases, while others’ conditions remain unchanged despite remaining in hospital.
For such people, we offer the option of home healthcare so that they can stay where they want. Home healthcare is offered to those who may have difficulty getting to a hospital or clinic, including those with terminal cancer, intractable neurological diseases, or dementia.

Applicable Home healthcare Patients 

Scheduled house calls are available to those whose condition, such as terminal cancer, intractable neurological disease, severe respiratory failure, cardiac failure, or dementia, make getting to a hospital or clinic difficult. We can provide terminal stage home care.

Terminal Care Experience

From January to December 2022, we handled 16 terminal care cases at the patients’ homes. Other terminal care cases were handled at the facility. We have handled over 100 terminal care cases to date. 

Visitation Area

Northern part of Saga City (basically north of the Nanbu Bypass), Kanzaki City, and Ogi City. We offer scheduled house calls to those within a 16-km radius of our clinic as the crow flies.

Importance of Home Healthcare

We believe that patients should be able to live out their lives as they wish, without pain or suffering.
Accordingly, at the beginning of a scheduled house call, a care conference and advance care planning (ACP) are held involving the patient and their family, with various professionals, such as a visiting nurse, pharmacist, and care manager, to discuss what is best for the patient.
We can also administer continuous subcutaneous infusion of medical narcotics if necessary to alleviate any pain.

22 Years of Accumulated Home Healthcare Knowledge and Experience

Over and above home healthcare expertise, palliative care – i.e. the knowledge and ability to "control pain" – is indispensable. We closely collaborate with doctors handling palliative care at the prefectural hospital on a routine basis, so we can help to control any pain, such as continuous subcutaneous infusion of medical narcotics via a syringe pump to sedation, under the guidance of specialists. We are also skilled at managing other symptoms besides pain.

We regularly participate in annual conferences organized by the Japanese Association for Home Care Medicine, and Japanese Society for Hospice and Home Care in order to ensure our home healthcare knowledge is fully up to date. We also contributed a few chapters, and supervised the editing for the publication of two guidebooks on home healthcare in Saga prefecture, namely the “Home Care Net – Saga”. These guidebooks can also be purchased at our clinic. 

Specialist knowledge and a wide-ranging network are indispensable for home healthcare.  We have access to a wide network of medical and welfare personnel nationwide as organizers of the Japanese Association for Home Care Medicine, Japanese Society for Hospice and Home Care, and Japan Association for Home Care Support Doctors.
