About this Clinic

Providing better Evidence-based Medical Care for Patients and their Families
We aim to provide medical care based on the latest medical knowledge.
As a personal physician, we are the point of first contact for any medical diagnosis and thus handle many different cases, but if a more detailed examination or further treatment is necessary, we consult with specialists at other medical institutions. Accordingly, the director gained a Doctorate in Medicine, wrote a medical dissertation for his doctorate, and has acquired numerous qualifications in his specialist areas as a certified medical specialist.
He has worked as a school doctor for a junior high school to provide community-based medical care as a primary care physician, and as a member of the community-based integrated care conference. He also helped to set up “Home Care Net – Saga” to promote community-based integrated care, as Saga prefecture had been slow to adopt home healthcare.
In order to strengthen medical cooperation throughout Saga, he has worked as an educator because the adoption of palliative care and communication seems slower than other regions. He has taught palliative care for the Saga Joshi High School Health Nursing Course for 14 years, and has also taught “cancer notification” at Saga University’s Faculty of Medicine for about a decade. As the saying goes, “All’s well that ends well,” he teaches the importance of communication in medical care, especially for terminal stage patients in the community.
Ordinarily, this should be done by the government, but “Home Care Net – Saga” voluntarily holds public lectures twice a year, and case review meetings every other month. As a result, “Home Care Net – Saga” has been highly evaluated nationwide.

Responsibilities as a Personal Physician
The Japan Medical Association defines a personal physician as a “Doctor who can give counsel about anything, is fully appraised of the latest medical care, can introduce appropriate specialists or professional medical organizations as necessary, and has a general ability to handle community medicine, healthcare, and welfare.”

Provision of Patient-centric Medical Care
In order to best provide patient-centric medical care, the director completed an instructor’s workshop to support decision-making while respecting the patients’ wishes in 2018, and completed a workshop on decision-making while respecting the patients’ wishes, and a counselor’s workshop for healthcare providers at homes and facilities in 2021, to provide better medical care for patients at the clinical site. He also provides training and lectures as an advance care planning (ACP) instructor nationwide.
Prioritizing Continuity in Provision of Medical Care
In order to prioritize continuity in the provision of medical care, we provide introductions and are introduced in cooperation with Saga University Hospital, Saga-ken Medical Center Koseikan, NHO Saga National Hospital, and JCHO Saga Central Hospital, etc. For the community, we participate in self-reliance support regional care meetings to give instruction at the “Otassha Honpo Yamato” community comprehensive support center in Yamato-cho.
We also provide training in community healthcare for trainee doctors of the Saga University Hospital, NHO Saga National Hospital, and NHO Ureshino Medical Center, and provide training to acquire qualifications as certified pharmacists under the Japan Primary Care Association.
Knowledge of palliative care to manage pain and discomfort is required for home healthcare, and the ability to handle dementia, network across multiple professions within our community, and cooperate with general hospitals, are essential in our aging society. Dr. Mitsuoka provides collaborative medical care for many patients. We provide instruction to many trainee doctors as a postgraduate clinical training facility for Saga University Hospital, NHO Saga National Hospital, and NHO Ureshino Medical Center. We also routinely collaborate with other doctors handling palliative care, such as at the Saga-ken Medical Center Koseikan.
Characteristics of this Clinic

Gastrointestinal Testing is Available (reservation required)
Ordinarily, reservations are required for upper and lower endoscopy, and abdominal ultrasonography.
Also, prior preparation is required at home starting in the early morning for lower endoscopy (colonoscopy), with arrival at the clinic at around 11:00 a.m., and the examination starting at around 12:30 p.m.
The examination takes about twenty minutes, after which one is recommended to rest until around 3:00 p.m. Elderly and other patients who require additional consideration for examinations are requested to come to the clinic at 8:30 a.m., so that all prior preparations can be done at the clinic.
Both upper endoscopy and abdominal ultrasonography examinations take about ten minutes, but the prior preparation time for the endoscopy is a further thirty minutes.
Tests for diabetes, blood sugar, hemoglobin A1c, and PT-INR levels (for patients taking warfarin orally) are available immediately at our clinic.

Collaboration with Saga University Hospital, Saga-ken Medical Center Koseikan, and NHO Saga National Hospital, etc.

This clinic is registered as a training facility for community medical care for Saga University and NHO Saga National Hospital, and provides instruction for trainee doctors. In addition, lectures on “cancer notification” and “terminal care” are provided as a part-time lecturer at Saga University.
This is also used as a training facility for universities, accepting trainee doctors on study placements, while the director also lectures at universities. We are a professional provider of palliative home care. Two major associations related to home care in Japan, namely the Japanese Society for Hospice and Home Care, and the Japanese Association for Home Care Medicine, are merging, with Dr. Mitsuoka as an organizer of the merged organization.
As Saga Chairman for the “Japan Society for Dying with Dignity,” the director is in charge of academic training at their headquarters. He is one of the foremost professionals in Japan in terminal care and the processes involved at the final stage of life.

As doctors, we must constantly strive to sincerely provide comprehensive and continuous support to our patients to fulfil our responsibilities as personal physicians who have earned their trust.

We strive to explain the diagnosis as clearly and simply as possible, while sharing the treatment objectives with the patient taking into consideration their lifestyle. If necessary, we will introduce appropriate medical specialists or institutions at the appropriate timing. In order to do so, we strive for better routine communication with other doctors in the community to support patients as a team.
〇 In order to reassure patients, a team of local doctors, led by a personal physician, will work together to support patients. For patients who may struggle to get to clinics as an outpatient, we will select the optimum way to support our patients, such as via home healthcare and online medical examinations.
〇 We strive to stay abreast of the latest medical technology, so as to identify the optimal treatment together with patients and their families.
〇 We respect the wishes of the patients, and work with the family to support the patient’s ability to enjoy their lifestyle with dignity.
〇 We provide vaccinations and health checks, health consultations to patients and their families across a wide range of life and work issues, and introductions to appropriate medical care as needed.
〇 We fulfill our role under the community-based integrated care system, such as issuing primary doctor’s opinions for patients, and introducing local care and welfare services in accordance with the patients’ requests.
